PT SERVICES
Chose from 3 different types of PT evaluations to suit your needs. A physician's referral is not needed to schedule an appointment unless required by insurance. Each evaluation is 90 minutes in length, allowing plenty of time for a thorough evaluation as well as treatment.
If you are a returning patient with a new ailment or it has been 3 months since your last visit, please schedule an evaluation before treatment.
If you are a new patient, realize that an evaluation is required prior to scheduling a treatment.
Costs below are reflective of patients not billing insurance. For patients utilizing insurance, fee schedules are unique to each plan.

For the first appointment you will need to print, fill out, and bring with you the following forms: Medical History, Consent to Treat, Consent to Communicate, Privacy Practices, and Financial Responsibility Agreement. Click here to access these documents. Hard copies are also available at the clinic and can be filled out upon arrival.
ORTHOPEDIC EVALUATION
90 minute evaluation includes patient history and therapy goals, postural assessment, gait assessment, functional mobility, range of motion, strength tests, balance and special tests. It also includes treatment such as Myofascial Release or other manual techniques, patient education or an individualized home program

PELVIC FLOOR EVALUATION
90 minute evaluation focused on health of the pelvic floor. A more detailed patient history is conducted along with postural assessment, functional mobility tests, hip, back and lower extremity range of motion, strength and special tests. Also may include (with patient consent) a vaginal exam of the pelvic floor to assess strength, muscle coordination, pelvic support system and pain. BALANCE AND VESTIBULAR (DIZZINESS)
EVALUATION
90 minute evaluation includes detailed medical history as well as specific tests to screen for balance, inner ear, central nervous system dysfunction and neck symptoms that may cause dizziness and feelings of "being off balance". Treatments may include the Epley's maneuver, balance retraining, posture education, head and neck Myofascial Release, patient education and a home program.

FOLLOW UP TREATMENT
60 MINUTES
One-on-one 60 minute treatment with a focus on Myofascial Release and may include patient education, retraining of movement patterns, stretches, strengthening and an individualized home program.

FOLLOW UP TREATMENT
90 MINUTES
One-on-one 90 minute treatment for patients that have more complex patterns of restrictions or chronic injuries and pain in multiple areas, for those patients traveling longer distances for treatment, or for those feeling like they need more hands-on treatment time.

FORMS OF PAYMENT
Currently accepting Regence, HMA, and Asuris insurance plans. If not billing insurance, payment is due at time of service. We accept HSA cards, credit cards, cash, and checks. Upon request, a superbill will be provided (except for Medicare patients) to submit to your insurance company for out-of-network benefits. Medicare will NOT reimburse for out-of-network services.
